This form is to insure that the puppy that you choose to make apart of your family dose not go to anyone else. The Deposit will Not be refundable. Please fill out the form with the puppies correct information to insure we are holding the correct puppy for you and submit it. You will receive a Receipt shortly after the form has been submitted to insure you that the puppy is legally your's.
